Please use current and evidence-based scholarly (peer reviewed) references to support the discussion, writing in academic (third person) language, (except when integrating personal reflections, as outlined below, where first person is appropriate). A minimum of 10 scholarly references must be used.You will need to:? Outline your case study’s scenario and circumstance (this should be person-centred and short – only approximately 250 words – remember to ensure confidentiality of the practice setting and use a pseudonym for your client).? Discuss assessments performed, identify problems and construct an appropriate plan of nursing care that is person-centred, age-appropriate and culturally sensitive.? Highlight any risks identified during the assessments performed (if assessed and no risks identified, discuss this). Explain the importance of proactive risk management relevant to the client, their situation, and to nursing broadly.
