The Healthy People 2030 goals led us to concentrate on diabetes in this academic study. Diabetes is a common chronic illness that requires patient education and self-management to maximize health. In this work, we create a patient scenario, a thorough patient education and training strategy, and an appropriate mHealth app to help our patients manage diabetes. Patient empowerment, self-care, and health outcomes for diabetics are the main goals. Uncontrolled diabetes affects millions globally and may cause serious problems. As healthcare experts, we must teach Sarah how to manage her diabetes. This study discusses the need for patient education and the possibility of mHealth applications to empower people to manage chronic illnesses like diabetes.
45 Sarah was diagnosed with type 2 diabetes, a life-changing disease. Sarah begins diabetes care with a strong desire to restore health. Her nursing evaluation shows a motivated, willing patient who wants to learn and participate in her treatment. Sarah has little diabetes information, including causes, symptoms, and risk factors. Meal planning and carbohydrate counting are difficult for her. Sarah also needs help understanding glucose monitoring and medication adherence. Glycemic regulation seems abstract to her. Despite these obstacles, Sarah is eager to learn. She wants to use technology to control her diabetes regularly. Her willingness to use technology offers a chance for comprehensive education and care. Based on a scholarly peer-reviewed article by Smith et al. (2021), which emphasizes the importance of patient education in diabetes management and the transformative power of technology to improve patient outcomes, we can create a patient education plan tailored to Sarah’s unique learning needs and readiness to use technology as a diabetes management tool.
Patient Education/Teaching Plan
Sarah’s patient education and teaching plan are tailored to her learning requirements and preparedness to use technology for diabetes control. This strategy intends to teach her type 2 diabetes management skills and information due to her enthusiasm and readiness to learn. Sarah started her studies by studying diabetic basics (Jeffrey et al., 2019). She will learn about her condition’s causes, risk factors, and consequences. She has to understand diabetic processes to appreciate proactive management. Dietary changes are key to diabetes treatment. Sarah will get detailed meal planning advice on balancing carbohydrate consumption with other nutrients. Sarah will also be encouraged to use the app’s educational materials and community support forum for inspiration. Sara’s understanding and application of information will be examined throughout her schooling. Open conversations and feedback sessions will help us resolve her issues, boosting her diabetes management confidence (Jeffrey et al., 2019).
