Sharing information and working together can improve patient outcomes by lowering the likelihood of medical mistakes, safeguarding patients’ rights, and boosting their health. To conduct this evaluation, I spoke with a nurse friend about a problem he’s encountered in a healthcare facility. During the conversation, he mentioned numerous issues, but technology was his main point of emphasis (Gallagher et al., 2023). Connectivity and the convenience of providing adequate care have both been greatly improved by technological advancements. It’s only sometimes the case, however. He explained that although the hospital had adopted technology well, patients still faced care fragmentation due to a need for integrated and coordinated treatment. The absence of a treatment plan and common patient history and information likely contributed to the lack of coordination and integration.

Evidence-based practice within an interdisciplinary team

It was clear from the conversation that the hospital was dealing with a fragmentation of the care it provided. Regarding this matter, a multidisciplinary strategy founded on solid data would be the most effective course of action. The use of interdisciplinary care plans is the method that would be most suitable for this situation from the perspective of working across disciplines. The hospital was feeling the effects of having disorganized treatment throughout the facility. As soon as the organization’s management became aware of the problems inside the company, they decided to provide training that would expound on the significance of change and foster multidisciplinary collaboration. The training was designed to ensure that the healthcare providers could provide coordinated care, effectively communicate information about patient histories, and implement care plans. It was essential for them to coordinate not just among themselves but also in multidisciplinary teams. He noted that while this did assist, there was still some resistance owing to the problems connected with the construction of successful multidisciplinary care plans. Some of these issues include disparities in interpersonal and communication dynamics.

Change management theories and leadership strategy

Good patient outcomes and high-quality care may be improved and maintained with the help of interprofessional teamwork. Effective change theories like the Lean Systems Approach and Lewin’s Three-Step Model for Change may guide multidisciplinary approaches to resolving organizational problems. By fostering a shared framework across all the healthcare professionals caring for a patient, these change management theories would be excellent in supporting the adoption of interdisciplinary care plans. By putting aside personal differences and working from a shared understanding, teamwork may be strengthened, and patient care can be improved. However, healthcare leaders are also essential. Shared leadership is vital at healthcare institutions to provide multidisciplinary solutions to organizational problems (Atherton et al., 2022). Two benefits of shared leadership are collective decision-making and developing interprofessional teams that work together to address any challenge. In this scenario, having the organization’s specialists report to the same management team would be beneficial, even if they all operate in separate divisions. This would guarantee that everyone is on the same page concerning expectations and duties and that the company runs smoothly. Interdisciplinary Issue Identification Assignment

Collaborative approaches

Integrating collaboration into day-to-day initiatives and fostering open communication are two collaborative practices that can help interdisciplinary teams work together more effectively. It’s also helpful to use technology as a foundation for cooperation tactics and as a hub for social engagement (Jones, 2023). The objective is to improve communication among the multidisciplinary teams by inspiring them to talk to each other informally. Collaboration relies heavily on open lines of communication. Therefore, when people get socially and professionally involved with one another regularly, their lines of communication open up. As a result, they can better collaborate on other pressing concerns, like fixing the company’s internal structure.
